To the best of our knowledge Medicare insurance does not cover hair prosthesis. However, if you have a secondary insurance carrier, you may collect from them. Before submitting to your secondary insurance carrier, call Medicare at 1-800-842-2052: Request a 1490 Denial Form. Have your red, white, and blue Medicare card available. After you receive the response from Medicare, you then pass the claim on to your secondary insurance carrier who may or may not cover the cost of your hair prosthesis. |
